Winding Down Your Program

Level 3 tasks can be completed in just 4 Easy Steps. Level 3 tasks are focused on winding down your program and submitting all the forms necessary for the program close. The following tasks must be completed in order to be eligible for Level 3 incentives. View instructions for each step here.

Step #1: Teen Empowerment Summit Registration.

Step #2: Submit original Sign-In Sheets and electronic Weekly Attendance Report.

Step #3: Submit post-test surveys.

Step #4: Submit updated Student Case Plans.

When you have completed all the steps above you may request your Level 3 incentive.
